Belong at Home Donnybrook provides services throughout the Donnybrook area from Boyanup and Capel down to Balingup.
Donnybrook branch was originally the Donnybrook/Balingup HACC service and joined Belong at Home in 2011
Belong at Home is a local community-based, not-for-profit organisation that provides home and community care support services to older people (over the age of 65 years) so they can continue to live independently and enjoy a quality of life at home for as long as possible. It provides Provides support services to frail aged or disabled, and their carers, to enable them to continue to live comfortably in their own home.
You may be eligible for Home And Community Care if:
- You are older and frail and having difficulty with everyday tasks.
- You have a disability.
- You are the carer of a frail older person or someone with a disability.
Belong at Home offers a wide range of services which we can adapt and tailor to suit your individual and specific needs. You might like to receive some help around your home, assistance with your meals, social outings, transport to your appointments, or assistance with personal care and clinical services.
Services Provided include:
- DOMESTIC ASSISTANCE: Cleaning and Shopping Assistance
- PERSONAL CARE: Showering, medication prompts etc. getting in and out of bed, basic foot care
- FOOD SERVICES: Meal assistance, learning to cook, nutritional education, menu planning preparing cooking and freezing meals, cooking for special diets
- MEALS ON WHEELS: Meals on Wheels delivery
- SOCIAL SUPPORT: Community Group outings, support to access the community, form filling, bill paying, telephone support, home visits, Sunday for Seniors, Men's Group
- CBDC: Social Groups Craft, Movies, Book Club, Scrabble Club
- HOME MAINTENANCE: Lawn mowing, pruning, weeding, odd jobs, smoke alarm checks
- TRANSPORT: Shopping , medical appointments, social activities, Perth transport medical
